Register as a New Patient.

Join us as a patient at St Martins Gate Surgery.

We want to welcome new patients joining us from Farrier House Surgery who live within our boundary area. If you would like to register with us, please use the postcode checker below and follow the instructions for registering. For more information, please visit www.farrierhousesurgery.co.uk/closure

We want to reassure our existing patients that the closure of this practice will not impact the service we provide to you—as practices across Worcester City Primary Care Network have worked alongside NHS Herefordshire & Worcestershire to mitigate the impacts of this planned closure.

All nine local practices are welcoming a share of Farrier House patients to our lists in line with their size and capacity, and extensive recruitment and planning have taken place across the city to allow us to ensure we can continue to offer a high standard of care to both our existing patients, and those joining us from Farrier House.

Register a New Patient

The practice provides personal medical services to patients who live within an agreed geographical area determined by the practice.

If you live within our area you can ask to be registered with the practice and, providing the practice list is not closed, you may be registered as a patient with the practice.

To check if your postcode falls within our catchment area, please use the postcode checker at the bottom of the map.

The practice will accept new patient registrations for those residents with a WR5 post code (excluding Kempsey & Littleworth). For clarification the practice will continue to register patients from the development on the former Norton Barracks site.

The practice will also accept new patient registrations from residents who live within a one kilometre area of Crowle Village School and also residents of the village of Broughton Hackett.

The practice will continue to accept new patients from Norton Hall Nursing Home which is just outside this area to maintain the agreement and commitment of the practice to Norton Hall Nursing Home.

All new patients registering with the practice will be requested to complete a medical questionnaire and registration form.

Temporary residents.

If you are not registered with a GP but need to see one, you can get emergency treatment at any GP Practice.

If you need treatment for more than 14 days, you will need to register with that GP Practice as a temporary or permanent patient.

You can register as a temporary resident if you plan to live near the GP Practice for up to 3 months. After 3 months you will have to apply to register with that Practice as a permanent resident.

You can register temporarily with a GP Practice while away from home for work, study or on holiday. You’ll remain registered with your permanent GP Practice.

What information do I need?

Try to have the following information with you when you see a temporary GP for the first time:

  • details of any medical conditions you have

  • details of medical conditions you’ve had in the past

  • the name of any medicines you’re currently taking

  • details of anything you’re allergic to

  • contact details for your permanent or previous GP surgery

  • your NHS number

Temporary Patient Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

There are some restrictions on registration for visitors from abroad. For more information on this visit NHS For Visitors From Abroad

Your registration will be processed within 48 hours. You will receive confirmation via email or text message of any further information that we require.

Disabled Patient Facilities

All medical facilities are fully accessible for disabled patients including lifts to the first floor. A portable hearing loop is also available for use by patients with hearing difficulties; please ask for this at reception.

Change of Address

Please let us know as soon as possible if your address changes. Use the postcode checker at the top of this page to check whether you are still in our catchment area.

You may use the online form in the “Managing your health” tab, to notify us of your change of details. If you don't wish to use the online form then drop into the practice and pick up a paper form. We would usually need patients to present to surgery to show proof of address, but in current times, proof via email will be acceptable. This must be an official document, ideally a driving licence, utility bill or similar.
