Patient Registration

registrationThe practice will accept new patient registrations for those residents with a WR5 post code (excluding Kempsey & Littleworth). Requests from residents where the surgery is the nearest surgery geographically to their home address may also be accepted. For clarification the practice will continue to register patients from the development on the former Norton Barracks site.

The practice will also accept new patient registrations from residents who live within a one kilometre area of Crowle Village School and also residents of the village of Broughton Hackett.

The practice will continue to accept new patients from Norton Hall Nursing Home which is just outside this area to maintain the agreement and commitment of the practice to Norton Hall Nursing Home.

All new patients registering with the practice will be requested to complete a medical questionnaire and registration form. An appointment will then be made with one of our Nurse Practitioners for a ‘New Patient Registration’ examination. This enables the nurses to carry out full health screening ie BP, weight, height, past and present medical history and family history.

Pre-Registration with the practice

In order to save time, you may wish to pre-register by clicking on the link below. When you have completed all of the details, click on the "Send" button to mail your form to us.  You will still need to come to the practice with your ID (see below) and sign the form in order to complete the registration process (provided that you live within our boundary area).  You will need to complete the registration process before you can book an appointment.

When you come to the practice to register or to complete the registration process, you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us.

We will require you to bring to the surgery proof of your residence at the address where you live, e.g. utility bill, driving licence.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

 


Alternatively you may print off a registration form, fill it out and bring it in with you, together with proof of ID,  before you will be able to book an apppointment.
